We are honored that you are considering us for your health needs. Sun Life offers continuous and comprehensive health care to the individual and the entire family. Our services include family practice and women’s wellness along with dental/orthodontic services. In addition to providing care when you are ill, we will also help you to achieve a healthy lifestyle and work with you to prevent illness. We look forward to establishing a great patient/provider relationship.
It is our goal to ensure that your visit(s) with us will be pleasant, and we always appreciate hearing how we did in meeting your expectations.
To speed up your registration process, please complete the registration and history form prior to arriving for your appointment, OR fax them to 520-836-2305 and we will have your information on file before you arrive!

In addition to these forms, please bring the following with you to your appointment:
- Health Insurance Card(s)
- Driver’s License
- Medical Records (if transferring from another provider)
- A list of all medications you are currently taking, including strength (VERY IMPORTANT). If you do not wish to list them out, you may bring them in a bag and our staff will sort them out for the provider
- Immunization record (for children)
If your insurance requires a Referral, please have your approved referral at time of visit.
VERY IMPORTANT - ANY APPOINTMENT - If you cannot keep your appointment, please contact us as soon as possible so we can reschedule your appointment for you.
All fees are due at time of service. We accept cash, check and credit cards
In Case of Emergency
If you feel you are experiencing a medical EMERGENCY, go to the hospital’s Emergency Department,
or call 9-1-1.
Before you call or visit please be sure to write down any question you have regarding your health. Please keep handy a list of ALL the medications you are currently taking. Please be sure to provide us with the most recent and correct insurance identification card - without proper information from you, the insurance may deny the claim, and you may be responsible for the charges.
