Community Outreach

Sun Life Family Health Center’s Outreach & Enrollment Specialists have been trained by the Arizona Association of Community Health Centers to provide application assistance and screening for state funded health insurance programs, including AHCCCS and KidsCare II. Our staff uses a web-based, streamlined process over the internet, which provides the applicant quicker processing (than paper application) and shorter wait times for application determination. And often there is no need for a separate interview with a DES or AHCCCS caseworker.

Sun Life Discount Plan

Sun Life provides quality health care to all people who use our services, regardless of their ability to pay. If you are uninsured, you may quality for Sun Life’s sliding fee discount. Sliding fee discounts allow patients to pay variable rates for services, depending on household size and income. Sun Life’s services available through sliding fee include:

    • Doctor Visits
    • Women’s Health, Maternity Care and Family Planning Services
    • Prescriptions at Sun Life’s pharmacy
    • Lab and X-Ray services
    • Dental Services
    • Orthodontics (Braces)

Community Health Needs Assessment

Sun Life Family Health Center continually evaluates existing and emerging healthcare needs of the target population and community to determine optimal allocation of health center resources and services throughout Pinal County. The Report of Findings from our most recent Community Health Needs Assessment for the Pinal County Service Area highlights the results from all evaluation activities conducted in 2012, including surveys, focus groups and a joint priority-setting meeting.
